Students wishing to participate in sports at Central must consent to regular COVID-19 testing using the following forms. Coaches must also participate in testing.
Any student athlete who tests positive for COVID-19 must isolate for at least 5 days. Following isolation, the following questionnaire must be submitted before returning to play.
